PanCareSurPass

Our overall aim is to leverage the digital transformation of healthcare to improve person-centred care for survivors of childhood and adolescent cancer by scaling up and implementing an interoperable Survivorship Passport (SurPass) v2.0 across Europe.

  • Collaboration

PanCareSurPass brings together experts in survivorship care, interoperability, databases, implementation science, prediction modelling, epidemiology.

  • Survivorship Passport

Clinics in Austria, Belgium, Germany, Italy, Lithuania and Spain will implement the digital Survivorship Passport into their Electronic Health Information Systems to improve person-centred care for survivors of childhood and adolescent cancer.

  • Communication

Sharing what we learn is an important part of PanCareSurPass.
